NOTE: The test items will be presented in English, with Spanish translations in parentheses after the English text. Please use the text written in the language with which you are the most comfortable.

This is not a "selection" test that one can either pass or fail. It is a descriptive test that will provide more in-depth insights into your typical style of doing things for the organization requesting this procedure.

As soon as you have completed the questionnaire, electronic notification will be sent immediately to the company requesting this procedure informing them that you have completed the process and that the results are available to them.

In completing this questionnaire, you agree with the following conditions:

• The results of this process will be transmitted only to a designated person or persons within the Company requesting this procedure.

• Your individual results are considered "confidential" and that information is not for sale or made available to any other organization or individuals outside of the scheduling Company.

• The information is considered the sole property of the organization scheduling the testing.

• All transmission of data will be "password protected" and conveyed over secure lines.

• You will not be provided with a copy of these results.

• The developers of this product are not responsible for any decisions made by the Company requesting your participation.
